Customer Credit Application

Note: you will need Adobe Acrobat® to open and complete the credit application. If you do not have Acrobat installed, you can download Adobe Acrobat Reader® for free from Adobe by clicking on the button below, but the Reader version of Acrobat will not allow you to save the file with information entered.

Instructions for credit application

1. Download the credit application to your computer by clicking on the link below.
2. A download window will open on your computer asking whether you would like to open or save the file. Select "Save File" and choose a location on your computer.
3. Open the file by double-clicking it or via Adobe Acrobat.
4. Complete all required fields. (You can type directly into the fields by clicking on them.)
5. Print and sign the completed credit application.
6. Fax the completed & signed form to (305) 593-0481, Attention: Credit Department.
7. Bank and Trade Reference Release forms can be completed in the same way.
